小儿日间手术麻醉中瑞芬太尼结合七氟烷的临床运用分析

摘    要:目的分析小儿日间手术麻醉中瑞芬太尼结合七氟烷的临床运用效果。方法 60例行日间手术的患儿,依据简单随机抽样法分为对照组和实验组,各30例。对照组行丙泊酚结合氯胺酮麻醉,实验组行瑞芬太尼结合七氟烷麻醉。对比两组不同阶段生命体征变化情况、苏醒情况及不良反应发生情况。结果两组麻醉诱导前、麻醉诱导即刻、手术开始时、手术5 min、手术结束时,心率、血氧饱和度、平均动脉压对比,差异无统计学意义(P>0.05)。麻醉诱导即刻、手术开始时、手术5 min、手术结束时,两组心率、血氧饱和度、平均动脉压均高于本组麻醉诱导前,但差异无统计学意义(P>0.05)。实验组苏醒时间(10.20±4.11)min、自主呼吸恢复时间(8.95±2.57)min、睁眼时间(7.86±2.30)min均短于对照组的(16.40±4.20)、(13.02±2.84)、(12.54±2.46)min,差异有统计学意义(P<0.05)。实验组不良反应发生率6.67%明显低于对照组的26.67%,差异有统计学意义(P<0.05)。结论小儿日间手术麻醉中瑞芬太尼结合七氟烷的对患者生命体征影响较小,术后苏醒快,且麻醉不良反应发生率较低,临床适用度较高,可优先选择。

Abstract:Objective To analyze the clinical effect of remifentanil combined with sevoflurane in anesthesia for pediatric day-surgery. Methods A total of 60 children with day-surgery were randomly divided into control group and experimental group, with 30 cases in each group. The control group received propofol and ketamine anesthesia, and the experimental group received remifentanil and sevoflurane anesthesia. The changes in vital signs in different stages, recovery and occurrence of adverse reactions were compared between the two groups. Results There was no statistically significant difference in heart rate, blood oxygen saturation,and mean arterial pressure between the two groups before induction of anesthesia, immediately after induction of anesthesia, at the beginning of surgery, at 5 min after surgery, and at the end of surgery(P>0.05). Immediately after induction of anesthesia, at the beginning of surgery, at 5 min after surgery, and at the end of surgery, the heart rate, blood oxygen saturation, and mean arterial pressure of the two groups were higher than those before the induction of anesthesia, but the difference was not statistically significant(P>0.05). The recovery time(10.20±4.11) min, spontaneous breathing recovery time(8.95±2.57) min, and eye-opening time(7.86±2.30) min of the experimental group were shorter than(16.40±4.20),(13.02±2.84), and(12.54±2.46) min of the control group, and the difference was statistically significant(P<0.05). The incidence of adverse reactions 6.67% in the experimental group was significantly lower than 26.67% in the control group, and the difference was statistically significant(P<0.05). Conclusion The combination of remifentanil with sevoflurane in pediatric day-surgery anesthesia has less impact on patients’ vital signs, quicker postoperative recovery, lower incidence of adverse anesthetic reactions, and higher clinical applicability. So it can be preferred.
