尖锐湿疣组织HPV感染基因芯片分型检测研究

摘    要:目的了解尖锐湿疣(CA)组织中感染HPV病毒亚型分布,CA治疗后原皮损部位HPV的携带情况及性病门诊HPV隐性感染的发生率。方法利用HPV基因芯片对1625例性病门诊就诊者进行23种HPV亚型检测。结果693例CA患者皮损组织HPV阳性率93.65%,共检出21种HPV亚型,其中低危型HPV6、11和43出现频率分别为51.15%、36.67%和6.93%,高危型HPV16、33、18、31、73、58和59出现的频率分别为14.79%、6.47%、6.00%、4.93%、4.77%、4.62%和3.85%,单一感染占67.49%,重叠感染占32.51%;189例CA患者治疗后半年内原皮损部位HPV的携带率50.26%,HPV58出现频率最高(24.21%);743例非CA患者HPV隐性感染率18.84%,共检出17种HPV亚型,低危型以HPV6、11和43为主,出现频率分别为24.29%、20.0%和7.86%,高危型以HPV16、18、58、35、33、56和59亚型为主,出现的频率分别为17.86%、10.00%、10.00%、7.14%、5.71%、5.71%和5.71%,单一感染占77.14%,重叠感染占22.86%。结论HPV11、6、16型感染是尖锐湿疣的主要型别,HPV58可能是最难治疗的病毒亚型,HPV隐性感染在性病门诊就诊者中较常见。

Detection and classification of the HPV genotype in outpatients with sexually transmitted diseases

Abstract:Objective To understand the distribution of subtypes of HPV in patients with condyloma acuminata(CA) and the positive rates of CA three or six months after treatment and the incidence of HPV in outpatients with sexually transmitted diseases.Methods There 23 genotypes of HPV in 1 625 outpatients with sexually transmitted diseases were determined by Gene-chips.Results The positive rates of HPV was 93.65% in 693 patients with CA,21 HPV genotype were detected and the incidence of low risk HPV genotypes 6,11 and 43 were 51.15%,36.67%and 6.93%;while the incidence of high risk HPV genotypes 16,33,18,31,73,58 and 59 were 14.79%,6.47%,6.00%,4.93%,4.77%,4.62% and 3.85%.The positive rate of single type infection with HPV was 67.49%,and the positive rate of mixed infection with HPV was 32.51%;The positive rate of HPV was 50.26% in 189 patients with CA 3 or 6 months after treatment.HPV58 was the most common HPV genotype;The positive rate of HPV was 18.84% in 743 patients with latent infection,17 types of HPV genotypes were detected including low risk HPV genotypes 6,11 and 43 with the incidence of 24.29%,20.0% and 7.86%,high risk HPV genotypes 16,18,58,35,33,56 and 59 with the incidene of 17.86%,10.00%,10.00%,7.14%,5.71%,5.71% and 5.71%.The positive rates of single types infection and mixed infection of HPV were 77.14% and 22.86%.Conclusion HPV 6,11 and 16 are the main genotypes in the pathogenesis of condyloma acuminata.;HPV58 may be the genotype most refactory to treatment among the 23 types of HPV genotypes;Latent HPV infection is common in outpatients with sexually transmitted diseases.
