一氧化氮对脑出血病人预后的影响

摘    要:目的探讨一氧化氮(NO)在脑出血后继发性脑损害中所起的作用及其对预后的影响。方法检测76例脑出血患者各个时期血浆NO、一氧化氮合酶(NOS)含量,并与66例健康人比较,分析上述指标在出血后的变化以及与出血量、格拉斯哥结果评分(GOS)的关系和手术对NO的影响。结果脑出血后1~3dNO、NOS含量明显高于4~7d、14d和对照组;脑出血的量与NO含量呈正相关;出血后1~3dNO越高,预后就越差;手术组与非手术组NO、NOS含量差异无统计学意义。结论NO参与脑出血后继发性脑损害的病理生理过程,动态观察脑出血病人的NO含量,有助于我们准确了解病情,指导临床治疗,并可以做为判断预后的一项重要指标。

Abstract:Objective To study the biological action of nitric oxide (NO) in the secondary injury and the effects of NO on for prognosis after cerebral hemorrhage.Methods Content of NO and activity of nitric oxide synthase (NOS) in plasma of 76 cases of cerebral hemorrhage were compared with those in normal control group.Correlations among variously periodic content of NO,activity of NOS,volume of cerebral hemorrhage and Glasgow Outcome Scale (GOS),and effect of operation on NO were analyzed.Results Compared with those in the control group,content of NO and activity of NOS in plasma of patients with cerebral hemorrhage in 1-3 day was significantly higher than those in 4-7 day,14 day and the control group.The volume of cerebral hemorrhage was positively related with content of NO in plasma.There was no significant difference between the operation group and the non-operation group.Conclusions NO is involved in the pathophysiology of the secondary injury after cerebral hemorrhage.Dynamic observation on the content of NO in plasma of patients with cerebral hemorrhage was helpful for clinical treatment,and the changes of content of NO could be an important index for prognosis.
