| 摘 要： | 目的:探讨困难腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)的手术方法,以有效预防并发症,降低中转开腹率。方法:回顾分析我院行困难LC 780例患者的临床资料,其中男604例,女176例。术前根据性别及B超等资料综合判断术中难易程度,分析造成手术困难的因素,以指导采用最佳手术方式及操作技巧减少中转开腹率。结果:本组LC均获成功,无中转开腹。其中Mirizzi综合征Ⅰ型16例和胆囊三角"冰冻样"改变49例均施行胆囊大部切除术。手术时间30~240min,平均69min。术后住院6~9d,平均7.8d,无并发症发生。结论:重视LC性别的影响因素,仔细解剖,严密操作,胆囊大部切除术残余的胆囊粘膜电凝烧灼,必要时"8"字缝合,充分引流,可顺利完成LC。
