从《内经》理论探讨中医对大肠癌的认识

《内经》中记载＂积之始生,得寒乃生,绝乃成积也＂,认为大肠癌的发生外因寒邪侵犯损伤脾胃,内因饮食不节、情绪不稳等各种因素引起正气虚弱,内因外因相互搏结成为息肉、溃疡等恶性病灶,正虚抗邪无力,癌毒蕴结,气血失调,脾胃亏虚而成。提出治疗大法为扶正祛邪,早期大肠癌以祛邪为主,晚期大肠癌以扶正顾护脾胃为主。临证应根据患者体质、证候进行辨证施治。

Discussion on TCM understanding of colorectal cancer from theories of Huangdi Neijing

This paper makes investigation about the original text of The Yellow Emperor＇s Inner Classic（ Huangdi Ne~ing ） related with colorectal cancer. The external cause is the attack of cold pathogenic factors on body to lead to spleen and stomach disease. The internal causes are various, such as improper diet and emotional lability, which affect the physiological functions of the spleen and stomach and eventually result in the accumulation of dampness （deficiency syndrome）. Internal and external injuries are often interacting with each other and become dysplasia （intraepithelial neoplasia）. It is difficult for healthy Qi to launch a struggle against pathogenic factors and eventually leads to deficiency syndrome and then the pathogenic factors become ＂deficiency-type pathogen＂. Colorectal cancer occurs due to attack of ＂deficiency-type pathogen＂. So, the basic treatment is to reintbrce healthy Qi and eliminate pathogen. Adjustments should be made for treatment based on syndrome differentiation, physical factors and symptoms of the patient.
