CT测量爆裂性骨折眼眶容积方法的选择

摘    要:目的:研究CT对于不同类型眼眶爆裂性骨折眼眶容积测量的最佳方法。方法:对眼眶下壁、内壁、内后壁单纯爆裂性骨折患者32例进行轴扫全眶测量、轴扫颧额突连线内测量和冠状面测量双侧眼眶容积,分别计算双侧眼眶容积平均值之差。结果:3种方法测量的双侧眼眶容积的平均值之差分别为:眼眶下壁骨折0.39ml、0.55ml和3.34ml;内壁骨折为4.63ml、1.32ml和1.02;内后壁骨折为2.25ml、2.26ml和2.05ml。结论:眼眶下壁骨折选择冠状面测量,其他两种测量方法因无阳性结果而被舍弃;内壁骨折选择轴扫全眶测量方法测量眼眶容积,其他两种方法,因测量范围不能完全包括骨折部位,测量值偏小而不用;内后壁骨折,3种方法测量结果相近,但轴扫增加患者的辐射和操作者的工作量,因此亦选择冠状面测量。

Abstract:Objective: To research the optimum method to measure orbital volume of blow out fracture of different types. Methods: In 32 patients with true blow out orbital fractures of different types, including medial wall fractures, orbital floor wall fractures and innerposterior wall fractures, Then to measure bilateral orbital volume by coronal scanning, axial scanning and from the zygomaticofrontal processes baseline to the tip of the orbit. Finally to calculate the difference of the average values of bilateral orbital volume separably.Results: The differences of the average values of orbital volume using three different methods were 0.39 ml?0.55 ml?3.34 ml in patients with blow out fracture of orbital floor. The differences of that were 4.63 ml?1.32 ml?1.02 ml in patients with medial wall fracture. The differences of that were 2.25 ml?2.26 ml?2.05 ml in patients with innerposteriorwall fracturs.Conclusion: 1.To choose coronal scanning in the patients with orbital floor fractures.2.To choose axial scanning in the patients with medial wall fracture.3. The results of three measurements using different methods were nearly equal in the patients with innerposterior wall fractures, but axial scanning increases radiation of patients and amount of work. Therefore should be choose coronal scanning to measure orbital volume.
