彩色高频超声对甲状旁腺腺瘤的诊断价值

目的探讨彩色多普勒超声(彩超)对甲状旁腺腺瘤的诊断价值。方法回顾性分析中国医科大学附属第一医院经手术病理证实的41例甲状旁腺腺瘤患者的彩超表现。结果彩超诊断甲状旁腺腺瘤32例,误诊2例,漏诊7例,诊断符合率78.0%;彩超对甲状腺下极病灶检出率高于上极(P<0.05),对左右两侧病灶检出率无差异;甲状旁腺腺瘤大小不等,边界清晰,其彩超图像多为类圆形低回声结节,内部回声较均匀,钙化灶和囊性变较少见;彩超可见丰富的血流环绕腺瘤或深入其内部,血流多来源于甲状腺下动脉;甲状腺疾病与异位甲状旁腺可影响彩超对甲状旁腺腺瘤的检出率。结论彩超是甲状旁腺腺瘤诊断及术前定位的可靠方法。

Value of color Doppler ultrasonography in the diagnosis of parathyroid adenoma

Objective To evaluate the value of color Doppler ultrasonography（CDU） in the diagnosis of parathyroid adenoma.Methods Collecting the materials of 41 operationed patients that were confirmed by pathology.All patients with parathyroid gland adenoma were examined with CDU and the CDU findings were correlated with operative and histological findings.Results The ultrasonography in diagnosis of 32 of the 41 lesionswere accurate,misdiagnosis were 2 lesions and missed diagnosis were 7 lesions.The diagnosis accurate rate of ultrasonography was about 78.0%;Lesions at lower pole of thyroid were more likely detected than that at superior pole（P0.05）,and there is no difference between the lesions at left and that at right lobe;Ultrasonography showed that parathyroid adenoma tended to be well demarcated,oval homogeneous hypoechoic mass with varying sizes,but calcification and cystic degeneration is rare;In color doppler image,these lesions had plenty of color signals within and around,most of these flow comes from inferior thyroid artery;Thyroid lesion and ectopic parathyroid may affect the detection of parathyroid in CDU.Conclusion It is suggested that CDU was reliable clinical value in diagnosis and localization of parathyroid adenoma before operation.
