经直肠超声在评估梗阻性无精子症中的价值

目的:研究经直肠超声在评估梗阻性无精子症患者中的价值。方法:梗阻性无精子症(OA)患者32例,非梗阻性无精子症(NOA)患者34例,正常生育男性30例,分别行经直肠超声检查,测量精囊的长径、前后径,比较三组间的指标有无显著性差异。结果:经直肠超声检查可获得清晰的前列腺、精囊结构图像,显示精囊与输精管壶腹、射精管、前列腺及膀胱后尿道的解剖关系;且OA与正常组、OA与NOA组间精囊的长径、前后径均有显著性差异(P<0.05),而NOA与正常组间无显著性差异(P>0.05)。结论:经直肠超声显示梗阻远端扩张的程度、梗阻的水平及与前列腺的关系优于输精管造影,有助于鉴别射精管梗阻的部位与原因,是目前诊断射精管、精囊及前列腺等男性不育相关病变首选的检查方法。

The evaluation of transrectal ultrasonography for patients with obstructive azoospermia

Objective:To evaluate the value of transrectal ultrasonography for patients with obstructive azoospermia (OA).Methods:66 azoospermic patients and 30 normal fertility males were included in this study. Of the 66 pafients, 34 were non-obstructive and 32 were obstructive(OA). All were subjected to transrectal ultrasonography. Results:Transrectal ultrasonography could show vivid images of prostate, seminal vesicle. The size of seminal vesicles in patients with OA was smaller than that in normal fertiles and patients with NOA (P<0.05). There were no significant differences between NOA and the normal(P>0.05).Conclusion:Transrectal ultrasonography is superior to seminal vasography in indicating extent of dilatation, level of obstruction and the relationship with prostates. It is helpful to distinguish location and reason of obstruction, and can be regarded as the first examination for male infertility diseases.
