白色念珠菌感染与义齿性口炎的关系研究

目的:研究口腔可摘义齿修复患者的义齿性口炎中白色念珠菌的致病性及控制义齿上白色念珠菌对治疗义齿性口炎的有效性。方法:2004-2007年40例可摘义齿修复后出现义齿性口炎患者,将初诊时白色念珠菌检查阳性的患者随机分为两组:对照组每日饭后常规刷牙漱口并清洁义齿,睡前将义齿浸泡于清水中;治疗组每日饭后刷牙漱口之后用3%碳酸氢钠漱口(100ml,分3～5次漱完),另嘱患者睡前用3%碳酸氢钠浸泡义齿。比较两组白色念珠菌菌丝量、黏膜颜色和自觉症状的变化。结果:全口义齿和局部义齿的白色念珠菌阳性率分别为87.5%和83.3%。可摘义齿修复后义齿性口炎患者的白色念珠菌阳性率85%。治疗2周和4周后治疗组白色念珠菌减少及自觉症状均较对照组明显好转。结论:义齿性口炎的发生与白色念珠菌感染密切相关,控制好白色念珠菌感染,可以减少义齿性口炎的发病率。

Study on correlation between infection of candida albicans and denture stomatitis

Objective:To explore the pathogenicity of Candida albicans in denture stomatitis and effects of elimi- nation of Candida albicans harboring denture.Methods:Forty cases with denture stomatitis from 2004 to 2007 were enrolled for study.The detection rate of candida albicans in preliminary visit was analyzed,and the positive cases were randomly divided into control group,who were instructed to use denture as usual,and therapeutic group, who were asked to rinse the buccal cavity with 3% sodium hydrocarbonate(NaHCO_3)solution and immerse the denture in NaHCO_3 solution over night.Specimen from denture surface was collected and smeared on glass slide. 10% potassium hydroxide was applied followed by warming up with small flame,and observed under microscope. Results:Positive rate of candida albicans in denture stomatitis was 85% among 40 patients,87.5% in patients with whole denture and 83.3% in patients with partial denture.The decrease in candida albicans and improvement of symptoms were obvious in the therapeutic group after 2-and 4-week treatment compared with the control group. Conclusion:The incidence of denture stomatitis is closely correlated with the Infection of candida albicans.Elimi- nation of candida albicans can reduce the incidence of denture stomatitis.
