SARS与细菌性肺炎不同病期的CT表现及动态分析

目的　探讨严重急性呼吸综合征 (SARS)与细菌性肺炎发病不同时期的CT表现及演变规律。资料与方法　对比分析 31例SARS患者和 34例细菌性肺炎患者的胸部CT资料。结果　SARS组早期多发小片磨玻璃影(4 / 7例 )多于肺炎组 (P <0 .0 5 ) ;进展期多发大片磨玻璃影 (2 2 / 2 5例 )、合并间质改变 (17/ 2 5例 )多于肺炎组 (P值均<0 .0 1) ;吸收期间质改变 (10 / 11例 )、合并磨玻璃影 (6 / 11例 ) ,多于肺炎组 (P值均 <0 .0 5 )。进展期两组病变部位分布明显不同 (P值均 <0 .0 1)。SARS组比细菌性肺炎组病变进展变化快 ,明显吸收时间长 (P <0 .0 1)。结论　SARS与细菌性肺炎发病不同时期的CT表现及演变规律显著不同

A Comparative Study and Follow-up on CT Features in Different Phase Between SARS and Bacterial Pneumonia

Objective To study the CT features and dynamic changes in different phases between patients with SARS and with bacterial pneumonia. Materials and Methods The data of chest CT scan of 31 cases with SARS and 34 with bacterial pneumonia were comparatively analyzed.Results In SARS group, comparing with bacterial pneumonia group, there were more multiple small patchy ground-glass opacities (4/7 cases) in early phase (P<0.05), multiple large patchy ground-glass opacities (24/25 cases) and accompanied with interstitial changes (17/25 cases) in progressive phase (P<0.01), and interstitial changes (10/11 cases) and ground-glass opacities (6/11 cases) in absorption phase (P<0.05). SARS differed greatly from pneumonia in lesion's location and distribution in progressive phase. SARS was appeared as more rapidly progressive, and more slowly resolving than pneumonia (P<0.01).Conclusion The CT features and the lesion's evolution in SARS are significantly different from that in pneumonia.
