Vitapex超填治疗难治性根尖周炎骨质破坏后的再生**

摘    要:背景:对根尖周骨质破坏较大的根尖周炎患牙,常规根管治疗的疗效不佳。目的:观察vitapex糊剂超填对根尖周破坏骨质的再生作用。方法:选择根尖周骨质破坏范围≥2 mm的恒牙根尖周病患牙120例,随机分为2组,氧化锌组进行常规根管治疗,vitapex组用vitapex糊剂超出根尖孔充盈骨质破坏区进行根尖骨诱导,定期复诊,X射线平片确认骨质破坏区有新骨沉积,去除多余的vitapex糊剂行永久充填。结果与结论:1年后复诊氧化锌组前牙43例,有效30例,后牙17例,有效11例;vitapex组前牙38例,有效36例,后牙22例,有效20例,两组相比差异有显著性意义(P < 0.05)。提示vitapex糊剂超填对根尖周骨质破坏范围≥2 mm的恒牙根尖周病损有良好的骨质再生作用。关键词:骨质破坏;vitapex糊剂;根尖周炎;超填;再生doi:10.3969/j.issn.1673-8225.2012.12.044

Abstract:BACKGROUND: Conventional root canal treatment for periapical periodontitis with periapical cementoclasia has poor efficacy.OBJECTIVE: To observe the effect of vitapex on bone regeneration beyond the apical foramen.METHODS: 120 patients with periapical periodontitis of the permanent teeth whose area of bone destruction was ≥2 mm were selected, and they were randomly divided into two groups: the zinc oxide group was treated with conventional root canal therpy, and the vitapex group was treated with vitapex beyond the apical foramen to fill bone destruction area. All the patients were required of regular referral. Then, deposition of new bone in bone destruction area was identified by using X-ray examination. At last, surplus vitapex paste was removed and permanent filling was conducted.RESULTS AND CONCLUSION: After 1-year referral, 43 incisors and 17 molars of the zinc oxide group were successful in 30 and 11 cases, respectively; 38 incisors and 22 molars of the vitapex group succeeded in 36 and 20 cases, respectively. The difference between the two groups was significant (P < 0.05). And vitapex is worthy of clinical application.
