胃癌全胃切除术后二种消化道重建术的比较

目的:探讨全胃切除后消化道重建方式的合理选择。方法:观察二种不同重建方法手术难度、术后病人的并发症及营养差异。结果:二种方法手术难度、术后并发症相差不太。P袢空肠Roux-en-Y重建术后早期适应性较好;功能性空肠间置吻合术后病人后期营养优于前者。结论:全胃切除后,P袢Roux-en-Y吻合术和功能性空肠间置吻合术在改善患者术后症状和生活质量等方面是比较理想的重建术式,但功能性空肠间置吻合术是一种更符合生理的术式,有条件,应优先考虑该方式重建消化道。

Two Species Comparison for the Digestive Tract Reconstruction after Total Gastrectomy of Gastric Cancer

Objective: Explore the reasonable choice of the digestive tract reconstruction after total gastrectomy.Method: To observe the difficulty of two different reconstruction surgery and the patient's postoperative complications and nutritional differences.Result: Two methods of operation and postoperative complications were no different.P limb Roux-en-Y reconstruction was good early postoperative;Functional Roux late nutrition was better than the former home anastomosis patients.Conclusion: P Roux-en-Y and functional jejunal loop between the home anastomosis in patients with symptoms and improving quality of life is an ideal type reconstruction after total gastrectomy,But functional Roux home anastomosis is a more physiological procedure,If conditions permit,priority should be given to reconstruction of the digestive tract.
