膀胱癌肉瘤2例报告并文献复习

目的结合文献复习，提高膀胱癌肉瘤的诊治水平。方法回顾分析2例膀胱癌肉瘤患者的临床和病理资料。结果1例行经尿道肿瘤电切术，术后病理示癌肉瘤，3个月后复发行根治性膀胱全切术；1例行根治性膀胱切除术，显微镜下为鳞癌和梭状细胞肉瘤成分，免疫组化显示CK及Vimentin反应均为阳性。结论膀胱癌肉瘤是一种高度恶性肿瘤，好发于老年男性，对分期较晚的病例，根治性切除是最佳治疗方案。膀胱癌肉瘤的预后与肿瘤分期密切相关，而与组织学表现关系不大。

Report two cases of carcinosarcoma of bladder and review the literature

Objective Combining literature review,to improve the diagnosis and treatment of bladder tumor. Methods Retrospective analysed clinical and pathological data of two patients with bladder tumor. Results One case showded transurethral tumor resection,postoperative pathology showed carcinoma sareomatodes,recurrence after 3 months with radical cystecto- my surgery ; 1 cases with radical cystectomy could be seen squamous cell carcinoma and spindle cell sarcoma components under microscope,and immunol histochemistry showed：CK and Vimentin reactions were positive. Conclusion CB is a highly malignant type of neoplasm,occur in elderly males redominantly. Radical operation appear to be the best treatment of choice in the advanced stage of the disease. The prognosis of CB is related to tumor stage and seems to be no relations with histologic appearance and outcome. Bladder cancer is a highly malignant sarcoma,and good fat in older men,on the late stages of cases,radical resection is the best treatment. The prognosis of bladder carcinosareoma is closely related with tumor stage,but has little to do with the histological features.
