血制品照射预防免疫功能低下血液病患者的输血后移植物抗宿主病

目的 :比较在免疫功能低下人群输入照射及未照射血制品后输血相关移植物抗宿主病 (AT- GVHD)发生率。方法 :采用回顾性分析方法 ,照射组病例中所有血制品均需经射线照射处理 ,总剂量 2 0 0 0 c Gy,剂量率2 0 0 c Gy/ min。结果 :未照射组 2 0 0例免疫功能低下患者中 ,发现 4例患有 TA- GVHD,发生率 2 .0 % ;照射组 197例免疫功能低下患者中无 1例发生 TA- GVHD。结论 :在免疫功能低下人群输注含淋巴细胞血制品时应去除和灭活淋巴细胞 ,照射可减少 TA- GVHD的发生率。

Irradiation of blood components to prevent transfusion-associated graft-versus-host disease in immunological deficient patients with hematological malignances

Objective:To compare the incidence of TA GVHD in patients with impaired immunity after transfusion with irradiated or unirradiated blood components.Method:Analyse retrospectively clinical features and outcome of the control group and the irradiation group,in the irradiation group,all blood components are irradiated by x ray,total dose is 2000 cGy,the dose rate is 200 cGy/min.Result:In the control group,four patients (2.0%) had TA GVHD,while in the irradiated group,no patient had TA GVHD.Conclusion:Lymphocyte in blood components should be inactivated or reduced when it is transfused to patients with impaired immunity,irradiation could reduce the incidence of TA GVHD.
