86例前置胎盘妊娠结局分析

目的　探讨前置胎盘的病因、相关因素及前置胎盘类型与妊娠结局的关系 ,寻找正确的处理方法 ,降低围生期病死率。方法　对前置胎盘 86例临床资料进行分析。结果　B超对前置胎盘的诊断率可达 97% ,前置胎盘最多见的相关因素是流产史、高龄、多胎次 ;前置胎盘的主要并发症为产前出血、早产、产后出血 ;前置胎盘的新生儿体重明显低于对照组 (P <0 .0 5) ;新生儿窒息率明显高于对照组 (P <0 .0 5) ;不同类型前置胎盘的初次出血孕周、出血次数、大出血例数和妊娠结局差异无统计学意义 (P >0 .0 5)。结论　前置胎盘妊娠结局有很大差异 ,没有典型的临床特征可循。前置胎盘确诊后适时以剖宫产结束妊娠为宜

RETROSPECTIVE ANALYSIS ON CLINICAL OUTCOMES IN 86 GRAVIDAS WITH PLACENTA PRAEVIA

Objective To investigate the relationship between causes of placenta praevia, relative factors, degree and clinical outcome,and to approach its management. Methods The data of 86 cases with placenta praevias were analysed. Results The accurate rate of placenta praevia by ultrasonography reached 97%;Placenta praevia was frequently related to abortion, hight old and twins pregnancy; Placenta praevia was frequently complicated by antepartum bleeding,premature delivery and postpartum hemorrhage; The birth weight and the rate of neonatal asphyxia in placenta praevia were significantly higher than those in control group(P<0.05);The gestational age at the first bleeding, times of bleeding, episodes of heavy bleeding, clinical outcome and the degree of placenta praevia in three groups showed no significant differences(P>0.05). Conclusion The clinical outcomes of placenta praevia are highly variable. There are no obvious clinical features that are typical in predicting the clinical outcomes. Once placenta praevia being diagnosed, cesarean section should be considered at the right time.
