肠内营养在老年多器官功能障碍综合征的应用

摘    要:目的:探讨肠内营养在老年多器官功能障碍综合征(MODS)患者救治中的效果和应用价值。方法:将我院近4年来收治的60例老年MODS患者随机分为EN组(30例)和PN组(30例),监测两组病人营养支持前后血糖、血常规、血电解质、肝肾功能、血清白蛋白、前白蛋白等指标,同时观察两组病人肠道二重感染和消化道出血的发生率、平均住院天数和营养支持费用等。结果:PN组营养支持第15天血糖、血清转氨酶、胆红素和血尿素氮显著高于EN组(P<0.05)。EN组各临床症状均优于PN组,平均营养支持费用明显低于PN组(P<0.01)。血清白蛋白、前白蛋白较营养支持治疗前均明显增高,两组比较无显著性差异(P<0.05)。结论:老年MODS患者及时应用营养支持能改善病人的预后,促进康复,而肠内营养有费用低、并发症少、实施方便等优点,更适于临床应用。

Abstract:Objective: To investigate and evaluate the effect of enteral nutrition given to the aged patients with multiple organs dysfunction syndromes(MODS).Methods: Sixty patients were divided into enteral nutrition(EN) group(n=30) and parenteral nutrition(PN) group(n=30).Fasting blood glucose,routine blood examination,serum electrolyte,aminopherase and blood urine nitrogen,blood creatinine,serum albumin and prealbumin of each group were detected and compared.Results: The level of blood glucose,and the liver and kidney function in EN group was lower than that respectively in PN group(all P<0.05).Symptoms and physical signs of patients in EN group were less or milder than those in PN group.There was no diffevence in the serum albumin and prealbumin between the two groups(P<0.05).Conclusion: Enteral nutrition applied to the elderly with MODS could improve the prognosis,promote the healing and decrease the incidence of complications,and is superior to parenteral nutrition.
