关节镜下微创治疗膝关节僵直31例

目的:观察关节镜下辅助髌外上切口联合松解治疗膝关节纤维粘连性僵直的疗效。方法:关节镜下关节内粘连带松解治疗严重膝关节僵直患者31例,其中7例辅助髌外上切口联合松解膝关节外侧粘连、挛缩带。术后早期全程持续被动运动功能锻炼。结果:31例患者术中伸膝0～5°,屈膝均超过120°,术后步态改善,跛行消失,无感染、皮肤坏死等并发症。结论:关节镜下与髌上外侧切口联合松解术保留了皮肤及关节囊的完整,创伤小,安全有效,术后并发症少,疗效较好。

Treatment of Stiffness of knee joint by minimally Arthroscopic lysis

Objective To investigate the methods of combined arthroscopic and mini-incision release for treatment of Stiffness of knee joint . Methods 31 patients with Stiffness of knee joint were arthroscopic arthrolysis, 7 cases of them were combined arthrospic and mini-incision release. Postoperative early stage CPM exercises were practiced throughout the whole course of the treatment. Results All patients achieved 0 to 5 degrees of extension and more than 120 degrees of flexion in the operation, The gait improved and claudication disappeared in all patients.The quadriceps strength reached grade Ⅳ～Ⅴ without infection and necrosis of skin. Conclusion Using this technique, skin and the joint capsule of knee was preserved completely, it is minimally invasive, safe and effective.
