16层螺旋CT对肝炎后肝硬化肝体积变化与肝功能分级的相关性研究

摘    要:目的 探讨肝炎后肝硬化肝叶体积变化规律及其与肝硬化严重程度间相关性,建立正常成人标准肝体积公式. 资料与方法 对101例经临床、实验室检查、B超及CT证实的肝炎后肝硬化患者(Child-Pugh分级A级39例,B级38例,C级24例)及113例正常对照者行16层螺旋CT扫描.采用仪器自身携带体积测定软件测量肝左外叶、尾状叶及全肝体积,计算平均体表面积全肝、左外叶、尾状叶体积、左外叶、尾状叶占全肝体积百分比、体积指数及体积变化率.由113例正常对照者身高、体重(BW)、体表面积(BSA)及全肝体积得出正常成人标准肝体积(LV)公式. 结果 正常成人标准肝体积公式为LV(cm3)=882.08×BSA(m2)-308.12或LV(cm3)=12.90×BW(kg)+437.91.正常人全肝体积为(1222.76±216.96) cm3.Child C级全肝体积及左外叶体积分别为(798.01±203.64) cm3和(213.04±74.84) cm3,明显<Child A、B级患者(P<0.05);Child A级尾状叶体积明显增大,为(36.83±22.11)cm3,与Child B、C级差异有统计学意义(P<0.05). 结论 16层螺旋CT能准确测量肝脏各叶体积;肝炎后肝硬化体积变化与肝功能分级有明显相关性,能较全面、准确地评价肝储备功能.
