早期康复联合心理干预对脑卒中后抑郁的影响

目的观察早期康复联合心理干预对脑卒中后抑郁(PSD)的影响。方法将188例PSD患者随机分为干预组92例和对照组96例。对照组采用常规康复治疗,干预组采用早期康复联合心理干预治疗。比较2组第2、4、8周汉密尔顿抑郁量表(HAMD)、美国国立卫生研究院卒中神经功能缺损评分(NIHSS)情况。同时比较2组第4周和第8周抑郁发生率。结果干预组第4周、第8周的抑郁发生率均低于对照组,差异有统计学意义(P<0.05或P<0.01)。干预组第4周、第8周的HAMD评分及NIHSS评分均低于对照组,差异均有统计学意义(P<0.05或P<0.01)。结论早期康复联合心理干预能降低脑卒中患者的抑郁发生率,改善PSD患者的预后,值得临床推广应用。

The impact of early rehabilitation and psychological intervention on the post stroke depression

Objective To observe the impact of early rehabilitation and psychological intervention on the post stroke depression（PSD）.Methods 188 cases patients with PSD were randomly divided into intervention group（92 patients） and control group（96 patients）.Control group were given routine early rehabilitation,intervention group were treated with early rehabilitation and psychological intervention.Compared the hamilton depression scale（HAMD）,U.S.national institutes of health stroke scores（NIHSS） after treatment for 2、4、8 weeks and the depression incidence rate after treatment for 4、8 weeks of 2 groups.Results The depression incidence rate after treatment for 4、8 weeks of intervention group were lower than that of control group,the difference was statistically significant（P 0.05 or P 0.01） ;the HAMD and NIHSS after treatment for 4、8 weeks of intervention group were lower than that of control group,the difference was statistically significant（P 0.05 or P 0.01）.Conclusion Early rehabilitation and psychological intervention can reduce the depression incidence rate,improve the prognosis of patients with PSD,and worthy of clinical application.
