Rockall危险性积分在护理急性上消化道出血患者中的应用

总结176例急性上消化道出血患者应用Rockall危险性积分系统进行护理的经验与体会.根据评分结果将患者分为低度危险组、中度危险组、高度危险组3组,采取相应的护理措施.对于低度危险组患者的护理注重疾病相关知识的宣教,使其认识到治疗与护理要求,从被动接受变主动配合,增进自护能力;对于中度危险组患者的护理注重病情观察,护理管理者安排足够人力值夜班,安置患者入住距离护理站较近的病房,使抢救物品处于备用状态;对于重度危险组患者注重保持呼吸道通畅、纠正低氧血症、保证血容量以及三腔二囊管的护理,并给予患者心理支持,护理管理者安排专人一对一护理患者,安置患者入住抢救室.本组176例急性上消化道出血患者运用Rockall危险性积分系统进行护理取得较好的效果,其病死率(13.1%)、再出血率(18.2%),较未运用Rockall危险性积分系统进行评估前的病死率(15.2%)、再出血率(20.3%)均有所下降.认为应用Rockall危险性积分系统后有利于提高护士对急性上消化道出血患者病情的正确判断,在观察、抢救及护理急性上消化道出血患者的过程中发挥重要作用;护理管理者根据Rockall危险性积分评估结果可合理安排人力及物力.
