腹腔镜胆囊切除术中胆管损伤8例分析

目的探讨腹腔镜胆囊切除术中胆管损伤的原因、预防和治疗。方法对我院2002年3月至2009年3月收治的2 482例腹腔镜胆囊切除术发生胆管损伤8例病人资料结合文献进行回顾性分析。结果 7例术中及时发现并中转开腹手术处理,1例术后3周后再手术发现。7例效果良好,1例反复发生胆道感染,效果差。结论腹腔镜胆囊切除术,胆管损伤应早发现,及时中转开腹手术,选用合适的手术方法处理。

Bile duct injuries following laparoscopic cholecystectomy

Objective To investigate the causes,prevention and management of the intraoperative injuries of the bile duct in laparoscopic cholecystectomy（LC.） Methods Among 2482 patients who were received a LC in our hospital from Match 2002 to Match 2009,8 cases of biliary injury were found and analyzed retrospectively.Results 7 cases of injuries were recognized and managed immediately during operation,and 1 case were found in re-operation after 3 weeks of LC.1 of them had repeatedly duct infection and the others were recovered and discharged from hospi-tal.Conclusion Early recognition of bile duct injury is of paramount importance,immediate recognition and suitable management are associated with improved outcome.
