18F-FDG PET纵隔淋巴结高代谢的分析

摘    要:目的分析亚临床感染和肺癌转移所致的18F-FDG PET纵隔淋巴结高代谢的不同特征.方法对300例健康体检者和15例非小细胞肺癌(NSCLC)纵隔淋巴结转移患者进行FDG PET显像,以肉眼和SUV相结合的方法对纵隔淋巴结进行分析.结果 9.67%的健康查体者见纵隔淋巴结浓聚,年龄51~90岁,淋巴结分布较规律, SUV 2.06±1.01;纵隔转移淋巴结分布规律性较差,SUV 3.16±1.34.结论肺癌转移和老年亚临床感染均可致纵隔淋巴结FDG摄取增高,须根据年龄、病史及淋巴结显像特征进行综合判断.

Abstract:Objective To analyze the differences of FDG uptake in the mediastinum lymph nodes between benign inflammatory and metastasis in NSCLC.Methods Whole body FDG PET imaging was performed on 300 normal subjects and 15 patients with mediastinal lymph nodes matastasis caused by NSCLC. SUVmax of the mediastinal lymph nodes were measured and were analyzed with visual inspection.Results The uptake of mediastinal lymph nodes were observed in 9.67% of normal subjects aged 51-90 years old. The lymph nodes seemed to be regular in size, shape and arrangement, and SUVmax was 2.06±1.01 in healthy elders. The uptake of matastatic mediastinal lymph nodes showed irregular size and shape,and SUVmax was 3.16±1.34.Conclusion The hypermetabolism of mediastinal lymph nodes can be caused not only by metastasis but also subclinical inflammation. Diagnosis should be made by analyzing the age, clinical history and the feature of FDG uptake.
