原发性脾脏良性肿瘤的诊断与治疗

目的探讨原发性脾脏良性肿瘤的诊断和治疗对策。方法回顾性分析27例原发性脾脏良性肿瘤的临床表现、影像学特点和疗效。26例患者行B超检查，其中23例行CT检查．2例行MRI检查。24例作脾脏切除术。结果原发性脾脏良性肿瘤临床表现无特异性，仅16例有非特异性临床表现。影像学检查多表现为脾实质内单发或多发的实性或囊性结节，B超定位诊断率为100％，CT、MRI对良性脾脏肿瘤的定性诊断率为90％以上，尤其适用于脾脏血管瘤和囊肿的诊断。脾脏切除术疗效好。结论原发性脾脏良性肿瘤的诊断关键在于提高警惕，并选用合适的影像学检查以确诊。其治疗首选外科手术。

Diagnosis and treatment of primary benign splenic tumors

Objective To explore the methods for diagnosis and treatment of primary benign splenic tumor.Methods The clinical features,imaging characteristics and the curative effects of 27 cases of primary benign splenic tumor were retrospectively analysed. Of the 26 cases receiving ultrasound examination,23 cases were tested by CT and 2 by MRI.Twenty-four cases underwent splenectomy.Results The clinical manifestations of the primary benign splenic tumor were not specific.Only 16 cases had the unspecific clinical manifestations.Its imaging characteristics mainly displayed single or multiple solid or cystic nodes.The localization rate of ultrasound was 100%,and the qualification rate of CT and MRI was more than 90% in the diagnosis of the benign splenic tumor,especially for the splenic angioma and cyst.Splenectomy had good results.Conclusion The key points in diagnosing primary benign splenic tumors are the vigilance to these diseases and choice of appropriate radiographic methods.Surgical management is the first choice for their treatment.
