17682例不同年龄老年高血压患者并发症发生率的比较

目的探讨不同年龄对老年高血压患者发生并发症的影响。方法选择老年高血压患者17682例,根据患者年龄分为1组(60～64岁,4 929例)、2组(65～69岁,4 695例)、3组(70～74岁,4 236例)、4组(75～79岁,2501例)、5组(80～84岁,949例)、6组(85～89岁,299例)和7组(≥90岁,73例)。回顾性分析临床和实验室资料。结果在1组、2组和3组高血压患者中,前4位并发症依次为糖尿病、脑血管病、心肌梗死、心功能不全(P0.05);从第4组开始,脑血管病位居并发症的第1位(P0.05);从第6组开始合并肾功能不全位居第4位(P0.05);第7组高血压合并心功能不全患者位居高血压合并基础疾病的第2位(P0.05)。结论脑血管病是年龄≥75岁高血压患者的首要并发症,对于年龄≥75岁的高血压患者还应积极防治肾功能不全和心功能不全。

Comparison of incidences of complications in 17 682 hypertensive patients of different ages

Objective To study the effect of age on complications in hospitalized aged patients with essential hypertension.Methods 17682 patients with essential hypertension were enrolled and devided into 7 groups:group 1(60 — 64 years old,4 929 cases) ,group 2(65 — 69 years old,4 695 cases), group 3(70 — 74 years old,4 236 cases),group 4(75 — 79 years old,2 501 cases),group 5 (80 — 84 years old,949 cases),group 6(85 — 89 years old,299 cases),group 7(≥90 years old,73 cases).The clinical and laboratory data of them were retrospectively analyzed.Results The first four complications in groups 1,2,3 were diabetes,cerebrovascular disease,myocardial infarction, cardiac insufficiency.The first complication was cerebrovascular disease in patients of groups 4,5, 6,7 (P< 0.05).Renal insufficiency was the fourth complication in patients of groups 6 and 7 and cardiac insufficiency was the second complication in patients of group 7 (P < 0.05).Conclusion The first complication is cerebrovascular disease in very old hypertensive patients and attention also should be paid to the occurrence of renal insufficiency and cardiac insufficiency in these patients.
