正畸助萌直立阻生下颌第二磨牙效果探讨

目的探讨错牙合畸形同时伴有下颌第二磨牙阻生的患者,采用不同治疗方案后的矫治效果。方法对13例错牙合畸形同时伴有下颌第二磨牙近中阻生的患者进行拍片定位及模型分析,根据畸形的情况确定采用直立阻生第二磨牙或是拔除阻生第二磨牙由第三磨牙替代两种治疗方案,进行正畸治疗。结果 13例病例共18颗近中阻生下颌第二磨牙中,12颗采用正畸装置直立下颌第二磨牙,治疗后均达到正常位置;6颗采用拔除后以第三磨牙替代的治疗方案,治疗后第三磨牙均已达到第二磨牙位置或明显近中漂移,且倾斜度良好,有较好的咬合关系。结论对于下颌第二磨牙近中阻生的错牙合畸形,外科暴露后的正畸助萌直立阻生下颌第二磨牙或拔除阻生下颌第二磨牙以下颌第三磨牙萌出替代,都是比较好的选择。

Primary Study of Treatment Plan Options for Mesially Impacted Mandibular Second Molars

Objective To explore the orthodontic treatment effect for malocclusion with mesially impacted mandibular second molars.Methods 13 mesially impacted mandibular second molars orthodontic patients were selected and X- Ray films were taken to determine their positions. Orthodontic treatments were carried out after choosing treatment plan between uprighting mesially impacted mandibular second molars and extracting impacted mandibular second molars. Results 18 mesially impacted mandibular second molars of 13 patients were uprighted well by orthodontic appliances. 6 of all the impacted teeth were extracted. The third molars achieved good positions after extraction of impacted mandibular second molars. Conclusion To mesially impacted mandibular second molars,both uprighting mesially impacted mandibular second molars and replacing impacted mandibular second molars by third molars are suitable options.
