不同浓度罗哌卡因对肺叶切除术竖脊肌阻滞效果的分析

摘    要:目的探讨临床上常用的两种浓度的罗哌卡因对肺叶切除术中竖脊肌神经阻滞效果的影响。方法选择择期行胸腔镜肺叶切除术的患者60例,采用随机数字表法分为2组,每组30例:A组予0.5%罗哌卡因30 ml,B组予0.375%罗哌卡因30 ml,在超声引导下将罗哌卡因注入单侧竖脊肌平面行竖脊肌神经阻滞。记录并比较两组患者术中镇痛药物(舒芬太尼)的用量、神经阻滞起效时间、神经阻滞时长、阻滞成功率及局麻药毒性反应的发生率。结果两组患者手术时间、术中舒芬太尼用量以及阻滞时长比较,差异均无统计学意义(P>0.05);两组神经阻滞起效时间相比,B组慢于A组,组间差异有统计学意义(P<0.05)。两组毒性反应例数比较,A组多于B组,差异有统计学意义(P<0.05)。结论肺叶切除术中使用0.375%罗哌卡因30 ml行竖脊肌阻滞起效稍慢,但阻滞持续时间较长,毒性反应发生率低,可作为首选方案。高浓度局麻药使用时可结合病人全身情况,考虑加入血管收缩剂联合使用。
