儿童后肾腺瘤1例并文献复习

目的:探讨儿童后肾腺瘤的临床特点及影像学特征,提高对后肾腺瘤的认识。方法:报道1例7岁女性后肾腺瘤患儿的临床资料,并复习相关文献进行总结。患儿因发现左肾占位病变入院,伴发红细胞增多症,CT见肿瘤边界清晰、质地均匀、高密度并伴有点状钙化,诊断为左肾肿瘤,行左肾切除。结果:术中见肿瘤被膜完整,切面色灰白,质地均匀,病理检查:后肾腺瘤。术后半月复查,红细胞计数、红细胞压积、血红蛋白恢复正常。结论:儿童后肾腺瘤术前难以依据影像学特征明确诊断,对于CT值高、边界清晰、点状钙化及伴发红细胞增多症的肾脏肿瘤,应作术前穿刺活检或术中冰冻切片,明确诊断后,力争行保留肾单位的肿瘤切除。

Metanephric Adenoma in Children:A Case Report and Review of Literatures

Objective:To investigcto the clinical and imaging characterstics of metanephric adenoma in chidren.Methods:A 7 years old girl with metanephric adenoma was reported and relevant literatures also reviewed.The predamind feature was mass of the left kidney,associated with polycythemia.CT scan showed displayed the mass with sharpness of border,homogeneous,high-density and punctate calcifications.Tumor of kidney was suspected and nephrectomy was performed on her.Results:The tumor was found integrity tegument,homogeneous and a grey cutting surface during operation.The pathelogicd findiy cofirmed metanephric adenoma.Red blood cell count,hemoglobin and haematocrit recovered half a month after operation.Conclusions:It is difficult to get the final diagnosis of metanephric adenoma only by imaging characters.If the renal neoplasm with high-density,sharpness of border,punctate calcifications and accompanied polycythemia,we should do the fine needle aspiration biopsy preoperatively or frozen section during operation.The nephron-sparing tumorectomy should performed after final diagnosis.
