摘    要:目的:探讨重庆地区0~3岁健康儿童25-羟基维生素D〔25(OH)D〕的参考值。方法:选取代表重庆市不同经济水平不同地域的区/县所在地医院儿保科进行正常体检儿童,按照纳入标准和排除标准,共纳入900例儿童,抽取静脉血,分离血清,采用高效液相色谱-质谱法(LC-MS)检测血清25(OH)D浓度,同时对每个调查对象进行体格检查及问卷调查。结果:重庆市0~3岁儿童外周血25(OH)D参考值范围为(34.99±20.27)ng/mL。女童和男童25(OH)D水平比较差异无统计学意义(u=-1.327,P=0.185);<1岁组儿童25(OH)D水平高于1~3岁组(u=2.062,P=0.040);冬春季25(OH)D水平低于夏秋季(u=-4.299,P=0.000)。结论:重庆地区0~3岁儿童25(OH)D的参考值范围为(34.99±20.27)ng/mL;健康儿童不同年龄、不同季节的25(OH)D水平有一定差异;本研究得出的参考值范围与全球专家共识及美国医学会的推荐值基本一致,可为婴幼儿期维生素D的补充及维生素D缺乏相关疾病的诊断提供参考。

Abstract:Objective: To probe into the reference values of 25-hydroxy vitamin D (25OHD) in healthy children aged from 0 to 3 years in Chongqing. Methods: According to inclusion and exclusion criteria, a total of 900 healthy children aged from 0 to 3 years with physical examination in child health care department of different hospital from districts/counties in different regions representing different economic levels of Chongqing were extracted. Venous blood was extracted, serum was separated, and serum 25OHD concentration was detected by high performance liquid chromatography-mass spectrometry (LC-MS). Physical examination and questionnaire survey were conducted for each respondent at the same time. Results: The reference values of serum 25OHD in children aged from 0 to 3 years in Chongqing was (34.99±20.27) ng/mL. There was no significant difference in the levels of 25OHD between girls and boys (u=-1.327, P=0.185). The levels of25OHD in the <1 year group were higher than those in the 1 to 3 year group (u=2.062, P=0.040). The levels of 25OHD in winter and spring were lower than those in summer and autumn (u=-4.299, P=0.000). Conclusion: The reference values of serum 25OHD in children aged from 0 to 3 years in Chongqing was (34.99±20.27) ng/mL. The 25OHD levels in healthy infants varied by age and season. The range of reference values obtained in this study is basically consistent with the global expert consensus and the recommended value of the IOM in the United States, which provides objective evidence for vitamin D supplementation and diagnosis of vitamin D deficiency-related diseases in infants.
