椎管内硬膜外海绵状血管瘤的临床及影像分析

目的探讨椎管内硬膜外海绵状血管瘤的影像学表现及对临床的诊断价值。方法6例均做了MRI检查，T1WI：TR400ms，TE12ms；T2WI：TR2700ms，TE106ms。均作了轴位、失状位、冠状位及11研增强扫描。结果6例患者中，位于胸段椎管内硬膜外者3例，位于颈段椎管内硬膜外者1例，位于颈胸段椎管内硬膜外者2例。发生于脊髓背侧者3例，其中2例环绕脊髓生长，呈“U”字形；2例位于脊髓左背侧。1例位于脊髓右背侧，有3例沿椎间孔向外延伸。肿瘤最小为2．5cm，最长23．6cm。相对应脊髓不同程度受压变细，硬脊膜均有受压征象。肿瘤信号强度在T1WI均呈等或略低信号，T2WI均呈高信号。增强扫描瘤体均匀强化及不规则环行强化。结论椎管内硬膜外海绵状血管瘤是罕见的良性肿瘤，MR的特征性表现对临床诊断提供了有价值的依据。

Clinical feature and imaging findings of spinal epidural cavernous hemangioma

Objective To discuss the clinical feature and imaging findingsof spinal epidural cavernous hemangioma.Methods Six pathologically proven cases of spinal epidural cavernous hemangioma were examined with MRI.Converntional and Gadolinium-enhanced MRI were performed in all cases.Results The lesions were at the thoracic spinal epidural level (n=3),cervical spinal epidural level(n=1)or cervical-thoracic spinal epidural level(n=2).Tumors were on the back of the spinal cord in three cases,and two of them surrounding the spinal cord formed "U" shape.In two cases,the tumors were on the left back of spinal cord.The tumor was on the right side of the spinal cord in one case.In three cases the tumors grew out through the intervertebal neural foramen.The shortest tumor was 2.5 cm,and the longest one was 23.6 cm The relative spinal cord was pushed aside.In all cases,the mass showed equal or relatively lower signal on T1WI,hyperintense on T2WI and homogeneous strong enhancement.Conclusion Spinal epidural cavernous hemangiomas are very rare benign tumors.MRI imaging supplies great value for the clinic diagnosis.
