选择性半肝血流阻断在肝癌半肝切除术中的应用

目的探讨选择性半肝血流阻断在肝癌半肝切除术中的临床价值和安全性。方法回顾性分析29例肝癌半肝切除术病例，讨论选择性半肝血流阻断在肝癌半肝切除术中的应用。结果20例患者手术均顺利完成，术后恢复良好，未出现肝衰竭并发症。结论对于适应证明确的肝脏疾病，通过选择性半肝血流阻断后行半肝切除术，可有效减少术中出血、降低术后相关并发症的发生。该术式又为肝癌半肝切除术提供了一个合理安全的术式，尤其是肝硬化患者，有利于术后肝功能恢复。

Clinical application of hemihepatic vascular control in hemihepatectomy for hepatoma

Objective To explore the clinical significance and safety of selective hemihepatic vascular control in hemihepatectomy for hepatoma. Methods Twenty cases of hemihepatectomy for hepatoma were reviewed. The application of selective hemihepatic vascular control in hemihepatectomy for hepatoma was discussed. Results Hemihepatcctomy for hepatoma was successfully performed, all patients recoverd well after the operation, and without related serious complications. Conclusions To indicated cases, hemihepateetomy for hepatoma with selective hemihepatic vascular control can be performed, which decreases intraoperative blood loss, preserves residual liver function, and decreases postoperative complications. Selective hemihepatic vascular control in hemihepatectomy for hepatoma is safe and effective to control in intraoperation bleeding, reduce complications and benefit the recovery of postoperatic hepatic function parameters.
