皮肌炎或多发性肌炎死亡因素分析

目的探讨皮肌炎或多发性肌炎（DM/PM）的死亡因素。方法收集近三年来在我院住院的121例DM/PM患者临床资料,采用Logistic回归进行回顾性分析。结果 16例DM/PM患者在住院过程中或自动出院后死亡,死亡率13.2%。单因素Logistic回归分析显示男性、肺间质病变、肺部感染、呼吸衰竭、恶性肿瘤与DM/PM患者死亡显著相关,肌病类别、年龄、病程、肌力、血清肌酶CK与DM/PM患者死亡无显著相关;多因素Logistic回归分析显示呼吸衰竭与DM/PM患者死亡显著相关。结论 DM/PM死亡率高,男性、呼吸系统受累、恶性肿瘤是DM/PM死亡的危险因素。

Analysis of cause of death in patient with dermatomyositis or polymyositis

Objective To investigate the cause of death in patient with dermatomyositis or polymyositis （DM/PM）. Methods Collecting medical records of 121 inpatients with DM/PM in our hospital during the latest 3 years,which were retrospectively analyzed by Logistic regression. Results Sixteen patients with DM/PM died in hospital or died only after they left from hospital and the death rate was 13.2%. Logistic regression analysis of univariate factor showed the conditions, such as male, interstitial lung disease, pulmonary infection,respiratory failure,malignant tumor, significantly correlated with death, however, types of myopathy, age, course of disease, musle force, serum creatase CK did not correlate with death. Logistic regression analysis of multiple factors showed that respiratory failure significantly correlated with death. Conclusion There are high death rate in patients with DM/PM. Male, respiratory system involvement and malignant tumor are possible risk factors of death in patients with DM/PM.
