纤维支气管镜在重型颅脑损伤患者治疗中应用的效果评价

目的:通过对150例重型颅脑损伤患者的回顾性调查,以探讨纤维支气管镜吸痰在重型颅脑损伤患者治疗过程中的临床意义。方法:将150例重型颅脑损伤患者按照是否应用纤维支气管镜吸痰分为两组:常规吸痰组50例,均采用一次性吸痰管吸痰;纤维支气管镜吸痰组100例,间断使用纤维支气管镜吸痰。结果:采用纤维支气管镜吸痰组肺部感染率为11%,常规吸痰组肺部感染率为34%,而气管切开率分别为38%与20%,纤维支气管镜吸痰组患者的住院天数、抗生素使用费用及住院总费用显著低于常规吸痰组。结论:纤维支气管镜在重型颅脑损伤患者中应用可显著提高气道及人工气道的管理效能,降低患者气管切开率及肺部感染的发生率。

Evaluation on clinical usefulness of bronchofibroscope in management of severe patients with traumatic brain Injury

Objective:It retrospectively compared bronchofibroscope assisted sputum aspirating versus conventional sputum aspirating methods in the management of severe traumatic brain injury.Method:150 patients were divided into two groups.Group one with 50 patients used conventional sputum a spiration method.Group two with 100 patients used the bronchofibroscope assisted method.Result:The pulmonary infection rate was 34% in group one versus 11% in group two.The rate of tracheotomy was 38% versus 20% respectively.The cost for antibiotic,total cost in hospital and days in hospital for group one versus group two.Conclusion:The application of bronchofibroscope assisted management of respiratory tract in treatment of severe traumatic brain injury may reduce tracheotomy and pulmonary infection,improve the oxygenic metabolism,and decrease the cost for respiatory tract care.
