吡嗪酰胺致严重高尿酸血症78例临床分析

目的:探讨吡嗪酰胺致严重高尿酸血症的影响因素及对抗结核治疗的影响。方法:对我院78例因应用吡嗪酰胺导致严重高尿酸血症的患者,从性别、年龄、体重、并发症、用药剂量、出现严重高尿酸血症的时间等6个方面进行分析。结果:吡嗪酰胺导致严重高尿酸血症的可能因素中:男性、年龄大于60岁者、体重指数≥28、合并糖尿病或高血压者易出现严重高尿酸血症,与对照组比较,差异有统计学意义(P<0.05);用药2周后出现高尿酸血症的几率增大(为80%)。虽然严重高尿酸血症发生率较高,但在临床治疗中大多数不需要停药。结论:应用吡嗪酰胺的体重肥胖、年龄偏大的男性患者及合并糖尿病、高血压的患者,出现严重高尿酸血症的发生率高。临床在用药2周左右时应密切注意严重高尿酸血症的发生。

Clinical Analysis of 78 Cases of Pyrazine Amide-induced Serious Hyperuricemia

OBJECTIVE:To investigate the influential factors of pyrazine amide-induced serious hyperuricemia and the effect of it on anti-tuberculosis treatment.METHODS:78 patients with pyrazine amide-induced serious hyperuricemia in our hospital were analyzed from gender,age,body weight,complications,drug dosage,the time of serious hyperuricemia appearing.RESULTS:Pyrazine amide-induced serious hyperuricemia was related with following factors:men,older than 60 years old,body mass index ≥28,hypertensive patients with diabetes suffered from hyperuricemia easily,compared with control group(P<0.05);the incidence of hyperuricemia increased after 2 weeks of treatment(80%).Although the incidence of hyperuricemia was in high level,but little case need to stop taking drug in the clinical treatment.CONCLUSION:Obese older male hypertensive patients with diabetes mellitus shows high incidence of serious hyperuricemia.Great importance should be attached to the occurrence of serious hyperuricemia within 2 weeks.
