腹腔镜改良式与常规大子宫切除术的比较

目的探讨在腹腔镜大子宫切除术中使用改良套扎方法的临床应用价值。方法2005年5月~2008年7月我院对163例大子宫良性疾病行腹腔镜筋膜内次全子宫切除术(laparoscopically intrafascial subtotal hysterectomy,LISH)或腹腔镜次全子宫切除术(laparoscopically subtotal hysterectomy,LSH),其中改良组85例,在术中边旋切子宫边收紧宫颈管套扎线圈,旋切完毕后打结;常规组78例,术中套扎宫颈管并打结后旋切子宫。观察2组患者手术时间、出血量及术后恢复情况。结果改良组手术时间:LISH(114.5±22.5)min、LSH(98.4±29.4)min,分别显著短于常规组LISH(166.3±45.1)min、LSH(146.2±32.4)min(t=-6.677,P=0.000;t=-7.034,P=0.000);改良组术中出血量:LISH(78.9±24.4)ml、LSH(69.7±22.3)ml,分别显著少于常规组LISH(211.7±45.6)ml、LSH(189.7±51.2)ml(t=-16.639,P=0.000;t=-13.784,P=0.000)。2组术后排气时间、切口愈合时间无显著差异(P0.05)。2组137例随访6个月,均无术后并发症发生。结论改良套扎的腹腔镜大子宫切除术中出血少、手术时间短、损伤小、并发症少,安全可靠,值得临床推广。

Comparison of Modified Laparoscopic Hysterectomy and Routine Hysterectomy for Gigantic Uterus
