人工股骨头置换治疗高龄股骨颈骨折

目的：观察人工股骨头置换治疗高龄股骨颈骨折的效果。方法：对75岁以上高龄移位型股骨颈骨折45例。实行人工股骨头置换。结果：本组无一例术中或住院期间死亡。全部45例平均随访18个月。疼痛消失，功能好，恢复生活自理能力36例，占80％；恢复部分生活自理6例，占13．3％，疼痛不能下地与生活自理3例，占6．7％。结论：人工股骨头置换治疗高龄股骨颈骨折效果满意。术前准备、麻醉和术中监护是手术成功及保证生命的关键，而术后护理及功能锻炼是患平稳度过围手术期和早日康复的重要措施。

Treatment of Femoral Neck Fracture in Advanced Age with Artificial Femoral Head Replacement

Objective:To observe the result of artificial femoral head replacement for treatment of femoral neck fracture in advanced age.Study the necessity of surgery,the importance of pre-operative and post-operative management.Methods:Artificial femoral head replacement was performed in 45 patients aged over 75 years.Result:No patients died during surgery,45 cases received follow-up for more than 18 months.Among them there were 36(80 %)cases in pan free well function and recovery of the ability in daily life care;6(13.3 %)in part recovery of the ability in daily life care;3(6.7 %)with poor result.Conclusion:Artificial femoral head replacement is effective for treatment of femoral neck fracture in advanced age.It is a key that operative preparation,anesthesia,monitoring during operation should be done.But it is also important that nursing and rehabilitation can improve condition and recover early after operation.
