常规超声联合超声弹性成像对颈部淋巴结疾病的诊断与鉴别

目的：探讨超声弹性成像在颈部肿大淋巴结良恶性鉴别诊断中的应用价值.方法：分析92例经病理证实的颈部肿大淋巴结的常规超声及弹性图像,计算淋巴结与邻近肌肉组织的弹性应变率比值,与病理诊断结果相对照,以受试者工作特征（ROC）曲线来确定弹性应变率比值的临界值.结果：良恶性淋巴结弹性应变率比值差异有统计学意义（P＜0.05）,临界值是2.265.以2.265为诊断界点,判断颈淋巴结良恶性的敏感性、特异性和准确性分别为87.18％、86.79％和86.96％.结论：超声弹性成像对鉴别颈部肿大淋巴结良恶性具有一定价值,但仍需与常规超声诊断相结合.

Conventional Ultrasound Combined with Ultrasonic Elaastography in Differential Diagnosis of Cervical Lymph Disease

Objective：To investigate the diagnostic value of ultrasonic elastography for differentiation of benign and malignant lesions in swollen cervical lymph nodes.Methods：92 swollen cervical lymph nodes confirmed by pathologic diagnosis were included in the study,both ultrasonic elastography and conventional ultrasonography were analyzed.Strain ratio of the lesion was calculated Compared with the patthoiogic diagnosis,the cut-off point of strain ratio was determined with receiver operating characteristic curve.Results：There was significant difference between the strain ratio of benign and malignant lesions（P＜0.05）.The cut off point of strain ratio was determined as 2265 sens tiv ty was 87.18％ and specificity was 86.79％.Conclusions：Ultrasonic elastography has certain value in differentiation of benign and malignant lesions in swollen cervical lymph nodes,but still needs combination with conventional ultrasound diagnosis.
