糖尿病患者食管pH值与血清胃肠激素相关性研究

摘    要:目的探讨糖尿病患者24 h食管pH值与血清P物质、胃动素、血管活性肠肽之间的相关性,为糖尿病食管病变的诊断及预后判断提供依据。方法选取我院住院确诊为2型糖尿病患者32例,同时设定对照组(同期我院健康查体者)32例。应用瑞典CTD-SYNECTICS公司生产的DigitrapperMKⅢ型食管pH值监测仪进行24 h食管pH值监测,应用ELISA法进行血清胃肠激素(P物质、胃动素、血管活性肠肽)浓度测定,通过检测糖尿病患者与正常人食管pH值及血清中胃肠激素浓度的变化,分析24 h食管pH值与P物质、胃动素、血管活性肠肽之间的相关性。结果糖尿病组与对照组之间SP、MTL、VIP存在显著性差异(P〈0.01),糖尿病患者24 h食管pH值与SP、MTL、VIP三者之间存在相关性,r值分别为0.780、0.478、0.379(P〈0.05)。结论糖尿病患者SP、MTL、VIP浓度的变化与24 h食管pH值变化存在密切相关性,三种胃肠激素可能共同作用,引起糖尿病食管病变的发生与发展;通过24 h食管pH值监测及血清胃动素、P物质、血管活性肠肽的联合检测,对糖尿病食管病变的诊断及治疗提供重要的参考依据。

Abstract:Objective: To detect the esophageal pH and the density of the serum gastrointestinal hormones(motilin;substance P;vasoactine intestinal peptide) between the patients of diabetes and the healthy by the analysis of the realationships between esophageal pH and serum gastrointestinal hormones and to provide a basis for diagnosis and prognosis of diabetic gastrointestinal.Methods: Thirty-two patients of types 2 diabetes and 32 healthy volunteers were selected in the Department of Endocrinology in our hospital.Esophageal pH detector was used for 24 hour esophageal pH monitoring.Patients’ serum gastrointestinal hormones concentration was measured by the application of Elisa.Statistical analysis of the correlation between 24h esophageal pH value and serum gastrointestinal hormones such as motilin,substance P,and vasoactine intestinal peptide was also made.Results: Statistics of the two groups showed remarkable differences between them(P<0.01).There was significant difference in two groups’s SP,MTL and VIP(P<0.01),and esophageal pH value was correlated with SP,MTL,and VIP.R values were 0.780,0.478 and 0.379(P<0.05).Conclusion: There are significant differences in 24h esophageal pH value and serum gastrointestinal hormones concentration between diabetic patients and healthy people.Esophageal PH change is correlated with the change of the serum gastrointestinal hormones(motilin;substance P;vasoactine intrestinal),the comprehensive evaluation of the diagnosis and the treatment effect can be made through joint test for diabetes esophageal lesions.
