改进Duckett术式治疗尿道下裂

目的 探讨改进横裁包皮带蒂岛状皮瓣尿道成形术（Duckett术）一期治疗尿道下裂的临床效果.方法 收集2010年2月～2011年1月应用改进Duckett术治疗的尿道下裂21例,依尿道口开口位置进行分类,分别为阴茎体型17例,阴茎阴囊型3例,会阴型l例,均有不同程度的阴茎下弯.同样收集2009年1月～2010年1月应用传统Duckeet术治疗的尿道下裂24例,分别为阴茎体型20例,阴茎阴囊型2例,会阴型2例,均有不同程度的阴茎下弯.会阴型均应用Duckett＋ Duplay术,不纳入研究.结果 本组病例均在术后随访超过半年.外观满意,尿道外口位于龟头正位,裂隙状,外观形似包皮环切术后.改进Duckett术并发尿瘘2例（9.52％）,包皮外观紫黑色0例,尿道口狭窄1例（4.76％）,尿道扩张后治愈.传统Duckett术并发尿瘘5例（20.83％）,包皮黑紫色伴结痂2例（8.33％）,痂自退后愈合,尿道狭窄3例（12.50％）.结论 改进Duckett术治疗尿道下裂效果满意,术后并发症发生率较低,且容易开展.

Treatment of Hypospadias with Duckett Improved Surgical

Objective To investigate the improvement of cross-cutting the foreskin pedicle flap urethroplasty （ Duckett technique） a treatment of the clinical effects of hypospadias. Methods From february 2010 to January 2011 Duckett application improvement of operation with one hypospadias cases were collected, number 21, in urethral mouth open position classification were ： the penis size of 17 cases, the penis scrotal type 3 cases, perineal type 1 patients, respectively, all had different degrees of the penis bent down, also collected in January 2009 to January 2010 Duckeet of surgical treatment with traditional one hypospadias case, number 24, were ： the penis size 20 cases, the penis scrotal type 2 cases, perineal type 2 cases, respectively, all had different degrees of the penis bent under. All application type perineal Duckett ＋ Duplay art, not included in the study. Results The patients were followed up for more than three months after .＂ surgery,appearance satisfaction, there was the shape of the penis after circumcision in the majority of cases. Urethral groove in the glans urethra anteroposterior, slit-like. Duckett improved after surgery complicated by fistula, 2 cases （9. 52% ） , the appearance of the foreskin purple 0 case, urethral stenosis 1 case （ 4.76% ） , rethral dilation basic cure. Tradi- tional surgery complicated by fistula Duckett,5 cases（ 20.8% ）, foreskin purple with black scab,2 cases（8.33% ）, callus from the back healing, urethral stricture,3 cases { 12.50% ）. Conclusion The modified Duckett in the treatment of hypospadias has satisfactory results, has a low incidence of postoperative complications, and is easy to carry out.
