内科胸腔镜在胸腔积液诊断中的应用价值

目的探讨对不明原因的胸腔积液患者应用内科胸腔镜进行诊断的临床价值。方法对2008年11月～2011年11月于笔者所在科室检查治疗的120例不明原因的胸腔积液患者临床资料进行回顾性分析,依据检查方法不同将其分为A、B两组,每组60例,A组应用胸腔镜进行检查,B组采取闭式的胸膜活检,对两组诊断的阳性率与术后并发症进行对比。结果 A组阳性患者56例(占93.3%),B组阳性患者28例(占46.7%),差异有统计学意义(P<0.05);A组患者并发症发生率为20.0%(12/60),B组患者并发症发生率为16.7%(10/60),差异无统计学意义(P>0.05)。结论对不明原因的胸腔积液患者应用内科胸腔镜进行诊断,不仅操作简单,而且安全快捷,同时还具有诊断率高以及并发症的发生率低等特点,应予推广。

The value of medical thoracoscopy in the diagnosis of pleural effusion

Objective To investigate the unexplained pleural effusion in patients with medical thoracoscopy diagnostic value.Methods A retrospectively analysis of the clinical data of 120 cases with the unidentified pleural effusion in the author's department from November 2008 to November 2011.According to the inspection method which was divided into A and B two groups,each group of 60 cases.Patients of group A took thoracoscope for inspection.Patinets of group B took closed pleural biopsy,compared with the diagnosis of positive rate and postoperative complications of the two groups.Results There were 56 positive patients(93.3%) of group A,28 positive patients(46.7%) of group B,the differences were statistically significant(P < 0.05).The incidence of complications of group A was 20.0%(12/60),that in group B was 16.7%(10/60),had no significant differences (P > 0.05).Conclusion Pleural effusion in patients with unexplained application of medical thoracoscopy for diagnosis,not only simple,safe and efficient,but also has characteristics of the diagnosis rate and a low incidence of complications,should be promoted.
