比较异丙酚复合芬太尼和地佐辛用于无痛人工流产的临床效果观察

摘    要:目的探讨异丙酚复合芬太尼和地佐辛用于无痛人工流产的临床效果。方法将120例自愿要求使用异丙酚无痛人流术的早孕妇女随机分为A组(60例,采用异丙酚联合小剂量芬太尼静脉麻醉),B组(60例,采用异丙酚联合小剂量地佐辛静脉麻醉),实施无痛人流。观察麻醉苏醒时间、镇痛效果,术中出血量、人流综合征的发生情况以及术后宫缩痛VAS评分和恶心呕吐不良反应。结果异丙酚复合芬太尼和地佐辛用于无痛人流均有镇痛作用,无痛率100%,两组镇痛有效率、苏醒时间比较差异无统计学意义(P>0.05)。异丙酚复合芬太尼呼吸抑制明显比异丙酚复合地佐辛组多,差异有统计学意义(P<0.05),苏醒后宫缩痛VAS评分地佐辛组明显低于芬太尼组(P<0.05),差异有统计学意义。结论异丙酚复合芬太尼和地佐辛用于人流均有良好效果,镇痛作用强,有宫口松弛作用,可降低人工流产综合征发生率,不增加阴道出血量。地佐辛复合异丙酚用于无痛人流手术具有血流动力学平稳、镇痛效果确切、苏醒迅速、麻醉期间不良反应及麻醉后并发症少等优点,其临床意义值得进一步探讨。

Abstract:Objective To investigate the efect of propofol combined with fentanyl and dezocine on painless induced abortion.Methods 120 early pregnancy women,undergoing induced abortion,were randomly divided into two groups:observed group(group B) undergoing the intravenous anesthesia by propofol combined with dezocine and control group(group A) receiving propofol and fentanyl.Observe recovery time after anesthesia,bleeding volume,induced abortion syndrome,pain of uterine contraction after operation,vomiting.Results Both groups did not show any significant difference in the analgesic effects and awaking time(P > 0.05).Compared with dezocine group,the rate of respiratory depression in fentanyl group was increased significantly(P < 0.05).Compared with fentanyl group,the VAS score of postoperative pain decreased significantly in dezocine group(P < 0.05).Conclusion Dezocine combined with propofol on painless induced abortion was the satisfactory and recovery faster,fewer side effects.
