切开复位钢板内固定治疗跟骨骨折临床分析

目的探讨跟骨骨折切开复位钢板内固定治疗的临床疗效。方法回顾性分析笔者所在医院2010年1月～2011年12月收治的跟骨骨折患者23例25足,采用切开复位钢板内固定治疗,术后1年随访并观察疗效,比较手术前后Bohler角及Gissane角度变化情况,采用Maryland评分评价效果。结果术后1年所有患者的Bohler角、Gissane角及Maryland评分与手术前比较,差异有统计学意义(P<0.05)。Maryland评分优16足,良5足,中4足,优良率达84%。结论术前全面评估骨折情况,掌握手术时机,术中严格规范操作,做到复位精确,术后及时有效处理并发症,辅以适当的功能锻炼,采取切开复位钢板内固定治疗跟骨骨折,可取到满意的疗效,明显改善患者预后。

Clinical analysis of open reduction and internal fixation in treating plate of calcaneus fractures

Objective To investigate the clinical effect of open reduction and internal fixation in treating plate of calcaneus fractures and collapse of posterior subtalar joint surface.Methods The clinical date of 23 cases 25 feet of calcaneus fractures adopted open reduction and internal fixation treatment in the author's hospital from January 2010 to December 2011 were retrospectively analyzed and the effect were observed in follow-up one year after operation.The Bohler and Gissane angle and the Maryland score evaluation results before and after operation were compared.Results There were significant difference in the Bohler angle,Gissane angle and Maryland score before and after one year operation (P < 0.05).The results of Maryland score were excellent in 16 feet,good in 5 feet,fire in 4 feet,the excellent and good rate was 84%.Conclusion To evaluate the condition of fracture preoperatively,choose the right operation time,standard operation,cure postoperative complications timely and effectively,do appropriate function exercise after operation,open reduction and internal fixation with plate of calcaneus fractures could take the satisfactory curative effect and improve the prognosis of patients.
