急性脑梗死患者首次发病时的血管造影分析

目的观察急性脑梗死患者首次发病时的脑血管狭窄病变比例、分布及其侧支循环状态。方法对16例急性脑梗死患者颈内动脉系统首次发病时行主动脉弓、高选择性颈部和颅内血管造影,使用血管直径判断血管狭窄程度,使用侧支循环分级判断其侧支灌注,分析血管狭窄病变的比例、分布及其早期侧支循环状态。结果16例急性脑梗死患者,数字减影血管造影术(DSA)正常1例,异常15例。共发现33处病变,平均每例患者约2处。10例(10/15)脑梗死发生后颅内侧支循环开放良好,患者症状较轻、恢复快;4例早期颅内侧支循环未开放,其中2例患者症状较重。结论DSA可明确脑梗死病变的性质、判断预后,为制定防治方案提供依据。

Angiographic finding in the first attack of acute cerebral infarction patients

Objective To observe the ratio and distribution of the involved arteries and the condition of col- lateral circulation in acute cerebral infarction after the first attack. Methods Sixteen patients suffering from acute ischemic stroke underwent cerebrovascular angiography of internal carotid artery system including the aor- tic arch and highly selective cervical and intracranial arteries to evaluate the degree of stenosis of blood vessels. The ratio,distribution of angiostenosis and the early condition of collateral circulation were observed.Results In 16 caseswith cerebral infarction,DSA revealed that 1 was normal and 15 were abnormal with 33 pathological lesions,averaging 2 pathological lesions each patient.Ten cases recovered rapidly with good condition of in- tracranial collateral circulation after acute ischemic stroke.Four cases did not have open collateral circulation, of whom 2 had severe clinical symptoms.Conclusion DSA can confirm the character of pathological changes, judge the prognosis and offer the evidence for fomulating the plans of prevention and treatment of isehemic stroke.
