高龄结直肠癌患者急性肠梗阻的外科治疗

目的探讨高龄结直肠癌急性肠梗阻外科治疗的选择。方法回顾性分析我院1995年1月至2005年12月间收治3l例65岁以上结直肠癌病人的外科治疗。结果3l例结直肠癌患者中，共发生术后并发症12例，其中在急诊手术后发生并发症8例，在择期手术病例中，4例发生术后并发症，急诊手术组围手术期死亡率为（2／13）15．38％，择期手术组围手术期死亡率为（1／18）5．56％（P〈0．05）。结论高龄结直肠癌患者急性肠梗阻需急诊手术时解除梗阻、抢救生命是首要考虑因素，手术方式可选择姑息性切除；对于结直肠癌并发急性肠梗阻非手术支持治疗有效，可限期手术治疗时。手术方式应尽可能施行根治性手术。

Surgical treatment of accuse intestinal obstruction of eld patients with colou or rectum cancer

Objective To investigate the choices of surgical treatment of accuse intestinal obstruction of eld patients who have colou or vectum cancer. Methods To retrospectively analyse the surgical treatments of the 31 patients above 65 years old who have colou or vectum cancer between January 1995 and December 2005 in our hospital. Results Among the 31 patients complications were occurred after operation in 12 patients and 8 were occurred after emergency operation. In the optive operation patients, 4 were occurred. The death rate of perioperation of the emergency operation group is 15.38% and the rate of optive operation group is 5.56%(P<0.05). Conclusion The chiefly consideration of the eld patients who have colou or vectum cancer suffering acute intestinal obstruction needing operation is to dissolve the obstruction and rescue life. Appensement cutting can be choosed. And if the sustain treatment without operation is effective limited operation can be choosed, and radical operation should be choosed as possible as we can.
