The value of PET-CT for etiologic diagnosis of secondary hemophagocytic syndrome

Objective: To research the presentation of secondary hemophagocytic syndrome(HPS) on PET-CT and the value of PET-CT for etiologic diagnosis of secondary HPS. Methods: Physical examination,laboratory tests and PET-CT for the four diagnosed secondary HPS patients were performed.Define the value of SUV greater than 2.5 as fluorodeoxyglucose(FDG) uptaken increased.Pathological biopsy were carried out when PET-CT showed malignant manifestation. Results: Four patients all presented uptaken increased FDG on lymph nodes and bone marrow.Among them,three patients presented the similar image on liver and spleen.Two patients didn’t have malignancy,their diagnosis was infection-associated HPS.Two patients showed FDG uptaken markedly increased and were diagnosed as lymphoma-associated HPS,and it was proved by histological diagnosis. Conclusion: The PET-CT images of patients with HPS showed that FDG uptaken increased on lymph nodes,liver,spleen and bone marrow,where the histocyte infiltrated.If PET-CT implied malignancy,the diagnosis was probably lymphoma-associated HPS.If not,it may be infection-associated HPS or others.
