肋骨外伤性骨折检出率差异与X线摄片技术相关因素分析

目的探讨X线摄片技术因素对肋骨外伤性骨折检出率的相关影响,提高对肋骨骨折的X线诊断准确率。方法分析850例胸部外伤致肋骨骨折行常规X线摄片检查,DR-PACS系统及CT三维重建检查的影像学表现。比较不同体位X线摄片方法 ,对肋骨骨折检出率的敏感性。结果肋骨骨折发生于右胸者521例,左侧329例,膈上肋骨骨折724例,膈下肋骨骨折126例,共检出肋骨骨折987处。常规X线摄片初诊检出肋骨骨折734例,漏诊116例,漏检率13.7%,多角度倾斜位改良法X线摄片检出肋骨骨折842例,检出率99.1%。结论常规X线摄片方法对检出肋骨骨折存在一定局限性,改进摄片体位和角度结合DR-PACS及多相位技术,CT三维重建检查可明显提高肋骨骨折的检出率。

Comparison between different detection of traumatic fractures or rib and technology of an X-ray examination

Objective To evaluate bound-up effect on technology of X-ry upon detdcting rates of traumatic fractures of rib to improve its correction of diagnosis.Methods The X-ray examinations,DR-PACS system and medical image by CT three-dimensional examination of 850 cases of fractures of rib due to traumatic injuries in chests were analyzed.Results There were 521 cases of fractures of rib in left chests,329 cases in left chests,724 cases of fractures of rib above diaphragm and 126 cases of fractures of rib below diaphragm,and to detect 987 catagmatic locations in ribs.734 cases of fractures of rib were preliminarily diagnosed by normal X-ray photography,while 116 cases are missed diagnoses,with 13.7% rate of missed diagnosis.824 cases of fractures of rib were detected by reformative X-ray examination in multi-angled and inclined positions with its rate of diagnosis of 99.1%.Conclusion Normal X-ray examination can not accurately detect fractures of rib,but it greatly increases the rate of diagnoses to reform the photographic technique to different positions and angles combined with the technology of DR-PACS,multi-phrase and CT examination of three-dimensional reconsruction.
