盐酸氨溴索持续气道湿化对慢性阻塞性肺疾病急性加重期机械通气的疗效评价

目的探讨盐酸氨溴索持续气道湿化对慢性阻塞性肺疾病(COPD)急性加重期机械通气的临床疗效。方法将60例建立人工气道的COPD急性加重的患者随机分为治疗组和对照组各30例,治疗组用盐酸氨溴索气管内持续微泵推注气道湿化,对照组用常规灭菌用水湿化液气道湿化,对比分析2组临床疗效、痰痂形成、刺激性咳嗽、气道出血及肺部感染发生率。结果盐酸氨溴索气管内持续微泵推注气道湿化可以提高临床疗效(P<0.05),减少痰痂形成(P<0.01),减少患者刺激性咳嗽(P<0.01),减少气道出血(P<0.01),降低患者肺部感染率(P<0.001)。结论对于COPD急性加重期机械通气患者应用盐酸氨溴索气管内持续微泵推注气道湿化效果明显优于灭菌用水湿化液气道湿化。

The clinical evaluation of continuous airway humidification with ambroxol on AECOPD patients undergoing mechanical ventilation

Objective To investigate the clinical effect of continuous airway humidification with ambroxol on AECOPD patients undergoing mechanical ventilation.Methods Sixty AECOPD patients with artificial airway were divided randomly into the experimental group(30 cases) and the control group(30 cases).The cases in experimental group were humidified with ambroxol continuously,and the control group was humidified with sterile water.The clinical effect,the formation of sputum crust,irritable cough,blooding in air passage and the rate of pulmonary infection were all compared.Results Continuous airway humidification with ambroxol can improve the clinical effect(P<0.05),reduce the form of sputum crust(P<0.01),incidence of irritable cough(P<0.01),blooding in air passage(P< 0.01) and lower the rate of pulmonary infection(P<0.01).Conclusion The effect of humidification with continuous Ambroxol is better than that of interrupted drop by sterile water on AECOPD patients.It can affect the airway directly so as to decrease the retention of mucous,increase the expulsion of sputum and improve the function of respiration.
