严重烧伤患者全身炎症反应综合征与器官功能不全的关系——全身炎症反应综合征评分的意义

目的：评价严重烧伤患者全身炎症反应综合征（ＳＩＲＳ）以及诊断ＳＩＲＳ的４ 个要素对器官功能不全的影响。方法：对严重烧伤患者诊断ＳＩＲＳ的４ 个要素进行评分，并与器官功能评分结果进行逐步多元回归分析。对不同诊断标准的ＳＩＲＳ发生时间与器官功能评分进行相关分析。结果：诊断ＳＩＲＳ的４ 个要素中，对器官功能不全的影响从大到小依次是呼吸频率、心率、体温和白细胞计数。严重烧伤患者不同诊断标准的ＳＩＲＳ发生时间均与器官功能不全的发生显著相关。尤其以满足３个要素的发生时间与器官功能不全评分的相关系数最高。结论：诊断ＳＩＲＳ的不同要素对严重烧伤患者预后的判断有不同价值，严重烧伤患者ＳＩＲＳ的持续时间以及严重程度影响器官功能不全的发生。

The relationship between systemic inflammatory response syndrome and organ dysfunction in severely burned patients:clinical significance of systemic inflammatory response syndrome score

Objective:To analyze the contribution of various diagnostic criteria of systemic inflammatory response syndrome (SIRS) to the development of organ dysfunction.Methods:Multiple regression analysis was done between the SIRS score and organ dysfunction score.At the same time,the relationship between the days for developing SIRS according to different criteria and organ dysfunction was also analyzed.Results:Four criteria of diagnosis had different contribution to the occurrence of organ dysfunction,and they ranked in following order:respiratory rate,heart rate,temperature,and white cell counts.In severe burn patients,the days for developing SIRS according to different criteria were significantly associated with the occurrence of organ dysfunction.Meanwhile severe SIRS was responsible for organ dysfunction.Conclusions:Each diagnostic criterion of SIRS bears a different prognostic value in serious burn patients,and the duration and seriousness of SIRS have a direct relation to the occurrence of organ dysfunction.
