食管结核9例及相关文献复习

目的:探讨食管结核的临床特点及诊疗方法。方法:回顾分析9例临床资料,男性7例,女性2例,年龄15-58岁。术前诊断为食管结核3例(纵隔内淋巴结结核2例,增生型食管结核1例);误诊6例,分别为食管癌4例、食管平滑肌瘤2例。药物治疗2例,手术治疗7例,包括行食管结核切除+胃代食管术2例、淋巴结清扫术4例、寒性脓肿切排术1例。结果:全部治愈,随访1-16年均无吞咽困难或结核复发。结论:食管结核发病率低,临床无特异征象,很难与食管癌和食管良性肿瘤鉴别。大多数病例采用药物治疗有效,若出现并发症者需手术治疗,术后仍需抗痨治疗12-18月,术后长期生存质量优良。

Diagonosis and Treatment of Esophageal Tuberculosis: Nine Cases Report and Review of the Literature

Objective: To discuss the experience of diagnosis and treatment for esophageal tuberculosis.Methods: The clinic data of nine patients with esophageal tuberculosis were reviewed.There were three cases of esophageal tuberculosis including two cases of medmediastinal lymph node tuberculosis and one case in hyperplasy-type.The other were diagnosised as carcinoma of the esophagus(four cases) and leiomyoma of esophagus(two cases).Two cases were cured with antituberculous chemotherapy,and the othe seen cases were operated.(Results:) All patients recovered well,with 1-16 years follow-up.Conclusion: Esophageal tuberculosis is rare and usually mistaken for the esophageal tumor,and most cases can be successfully treated with chemotherapy,or with different types of surgery if necessary.
