纤维蛋白粘合剂预防肠吻合口瘘的实验研究

摘    要:目的 观察小肠切除、肠吻合后局部用国产纤维蛋白粘合剂在促进吻合口愈合、预防吻合口瘘中的作用。方法 实验动物随机分为两组,对照组兔在肠切除吻合后表面喷涂生理盐水,实验组喷涂纤维蛋白粘合剂。术后第 1、 3、 7、 14 d切取含吻合口的肠管,分别测定肠吻合口破裂压、吻合口组织羟脯氨酸和胶原量。结果 术后两组动物均无吻合口瘘,术后第 1、 3 d,实验组兔肠吻合口破裂压高于对照组(约 5 mm Hg),局部肠组织羟脯氨酸、胶原量也多于对照组,但差异不显著。结论 纤维蛋白粘合剂可促进成纤维细胞增殖和胶原合成,对预防吻合口瘘有一定意义。

Abstract:Objective To evaluate the effect of fibrin sealant used at anastomotic site for preventing intestinal anastomotic leakage.Methods Five- centimeter segments of rabbits ileum were resected,then all animals were divided into two groups.In control group,normal saline was used on the surface of anastomotic site,meanwhile fibrin sealant was used in experimental group.Relaparotomy of all rabbits was performed in day 1,3,7,14 postoperatively. Anastomotic bursting pressure(ABP),hydroxyproline and collagen content of the tissue at anastomotic site were measured. Results The ABP of control group was about 5 mmHg lower than that of experimental group in day 1 and 3 postoperatively. The hydroxyproline concentration of the tissue at anastomotic site increased in experimental group compared with the control group. In experimental group,collagen content at the anastomotic site increased slightly in day 3 and 7. Conclusion Fibrin sealant can facilitate fibroblast proliferation and collagen synthesis to promote anastomosis healing.
