糖化血红蛋白与冠状动脉病变的相关性研究

摘    要:目的:探讨冠心病患者冠状动脉病变程度与血清糖化血红蛋白(HbA1c)水平之间的相关性及其临床意义。方法:387例患者按照冠状动脉造影结果分为冠心病组及非冠心病组。所有患者均采用放射免疫法测定HbA1c,根据住院期间冠状动脉造影结果,采用冠脉狭窄程度积分(Gensini评分)统计冠脉狭窄程度,作为冠脉病变程度的指标。结果:冠心病组的HbA1c水平较非冠心病组高,差异有统计学意义(P<0.01)。随着冠状动脉病变支数增加,冠心病组HbA1c的水平逐渐升高(P<0.05);随着Gensini评分的增高,冠心病组HbA1c的水平也逐渐增高(P<0.01)。冠心病患者冠状动脉病变支数与HbA1c呈正相关性(r=0.44,P<0.01);Gensini评分与HbA1c呈正相关性(r=0.46,P<0.01)。结论:检测HbA1c水平升高是冠心病的独立危险因素。

Abstract:ToexplorethecorrelationbetweenthelevelofglycosylatedhemoglobinA1c(HbA1c)andtheextent of coronary lesions in coronary heart disease. Methods:Three hundred and eighty-seven coronary heart disease patients were included in the study:coronary heart disease group and non coronary heart disease group. The levels of HbA1c of all the pa-tients were analyzed by radio immunoassay. All the patients underwent coronary angiography and the extent of coronary lesions was analyzed by total amount of coronary narrow degree integral( Gensini score). Results:Coronary heart disease group,non coronary heart disease group. The levels of HbA1c in coronary heart disease group were significantIy higher than those of non coronary heart disease group(P﹤0. 01). With the increase of coronary artery lesion,the levels of HbA1c in coronary heart disease group increased gradually(P﹤0. 05). With the increase of Gensini score,the levels of HbA1c in coronary heart dis-ease group were also increased(P﹤0. 01). Correlation analysis showed that the level of HbA1c was positively correlated with the number of coronary artery disease in coronary heart disease patients(r=0. 44,P﹤0. 01),the level of HbA1c was positively correlated with the Gensini score in coronary heart disease patients(r = 0. 46,P ﹤0. 01). Conclusion:The higher level of HbA1c was the independent dangerous factor for coronary heart disease.
